2.8.3 Personnel Records Personnel records of all Garnet A. Wilson Public Library employees are kept in the offices of the Director and the Fiscal Officer. These records are the property of the Library and are kept on file according to the Library's retention policy. Access to an employee's record is through the Director. An employee's personnel record may include the following materials:

  1. Application for employment and forms relating to hiring
  2. Requests for leave of absence
  3. Forms relating to reassignment
  4. Performance evaluations
  5. Reports of disciplinary actions
  6. Commendations, letters and memoranda relating to performance
  7. Records of attendance or tardiness at work
  8. Records of attendance at workshops, conferences and academic coursework that is job related
  9. Any other appropriate information pertaining to an employees position and/or job performance

I-9 Forms for employees are filed separately and are housed in the office of the Fiscal Officer. 

Payroll records and benefits information may be housed in the office of the Fiscal Officer. 

Confidentiality of Employee Personnel Records: 

The Board of Trustees of The Garnet A. Wilson Public Library of Pike County recognizes their responsibility concerning Personal Information Systems pursuant to Ohio Revised Code Chapter 1 347. Inquiries concerning personnel information must be referred to the Library Director for processing as authorized by ORC Chapter 1347 and, as necessary, in consultation with the Library legal counsel. Likewise, inquiries that may be related to Ohio Revised Code Section 149.43 must be referred to the Library Director and may be reviewed with Legal counsel. 

Issues relating to health are maintained in a separate confidential file.
